Student Data and Release Form Agreement
All University of Nebraska-Lincoln School of Music and Dance students and all non-major members of ensembles must complete and submit a personal data and release form. This form is due no later than September 1, 2007
Use your Blackboard user name and password to gain access to the web form. Students 19 and over must fill in the information completely and hit the submit button.

Students who are 18 or younger must download the pdf form, complete the information, obtain a parent or guardian's signature and return the form to the School of Music Office (room 113).
