Student Data and Release Form Agreement

All University of Nebraska-Lincoln School of Music and Dance students and all non-major members of ensembles must complete and submit a personal data and release form. This form is due no later than September 1, 2009

Use your Blackboard user name and password to gain access to the web form.

Students under the age of 19 must also have a parent or guardian download and sign a pdf.
